Allopurinol
Indications/Uses
Primary hyperuricemia (gout). To prevent uric acid & oxalate Ca deposition in secondary hyperuricemia. Excessive production of uric acid eg, malignancy, polycythemia vera, cytostatic therapy.
Dosage/Direction for Use
Adult Initially 100-300 mg daily. Maintenance therapy: 200-600 mg daily. Max single dose: 300 mg, may be increased to 900 mg daily, if necessary. Mild condition 2-10 mg/kg/day or 100-200 mg daily. Moderate condition 300-600 mg daily. Severe conditions 700-900 mg daily. Patients w/ CrCl >20 mL/min Normal dose, CrCl 10-20 mL/min 100-200 mg daily, CrCl 2-10 mL/min 100 mg daily or w/ long term interval. Childn 10-20 mg/kg daily or 100-400 mg.
Administration
Should be taken with food: Take after meals to reduce GI irritation.
Contraindications
Hypersensitivity. Acute gout attack.
Special Precautions
Effect can be reduced by salicylic & uricosuric agent eg, probenecid. Discontinue when skin rash or allergic symptoms occur. Avoid in patients w/ impaired kidney function or asymptomatic hyperuricemia. Perform hepatic function test periodically in patients w/ impaired hepatic function during the early stages of therapy; if itching, anorexia or wt loss occur. Increase fluid intake during use to avoid occurrence of renal stone. May impair ability to drive & operate machinery. Pregnancy & lactation.
Adverse Reactions
Hypersensitivity symptoms eg, exfoliative, fever, lymphadenopathy, arthralgia, eosinophilia. Skin reactions: Pruritus, maculopapular rash. GI disturbances, nausea, vomiting. Headache, vertigo, drowsiness, eye disorders, taste disturbances. Blood disorders: Leucopenia, thrombocytopenia, hemolytic anemia, aplastic anemia.
Drug Interactions
May increase toxicity of cyclophosphamide & other cytotoxic agents. May inhibit hepatic metabolism of drug eg, warfarin. May increase the effect of azathioprine & mercaptopurine. May prolong t½ of chlorpropamide & increase the risk of hypoglycemia, especially in patients w/ impaired kidney function. Reduced effect w/ salicylate & uricosuric agent eg, probenecid.
