Carbamazepine
Indications/Uses
Grand mal epilepsy, complex partial epilepsy (psychomotor or temporal lobe epilepsy) & combinations w/ epilepsy except for petit mal. Idiopathic trigeminal neuralgia, trigeminal neuralgia caused by multiple sclerosis, & idiopathic glossopharyngeal neuralgia.
Dosage/Direction for Use
Epilepsy May be given as monotherapy. Adult & childn >13 yr Initially 100-200 mg once or twice daily, increase gradually until optimum response is achieved (generally 400 mg 2-3 times daily). Childn 6-12 yr Initially 100 mg daily in 2 divided doses, increased 100 mg daily in 3-4 doses for each interval 1 mg until optimal effect is achieved. Not to exceed 1,000 mg daily, <6 yr Initially 20-60 mg in divided doses, may be increased to 60 mg daily every 3-7 days (steady state is usually <3 days) until optimal effect is achieved. Not to exceed 600 mg daily. Maintenance: Childn 11-15 yr 600-1,000 mg daily, 6-10 yr 400-600 mg daily. Not to exceed 1,000 mg daily. Trigeminal neuralgia Initially 200-400 mg daily in 2 divided doses to be increased to 200 mg up to 800 mg daily in divided doses to obtain pain relief. Larger dosage should be given in 3-4 divided doses. Max: 1,200 mg daily, reduce dosage gradually to a min effective dose as soon as pain can be overcome.
Administration
Should be taken with food.
Contraindications
Patients w/ impaired AV conduction & liver function, history of bone marrow depression. Concomitant use w/ MAOIs.
Special Precautions
Severe CV disease, history of haematological disorders. Hepatic or renal disorders. Elderly.
Adverse Reactions
Appetite loss, dry mouth, nausea, diarrhoea or constipation, headache, dizziness, somnolence, ataxia, confusion, agitation, visual accommodation disorders.
Drug Interactions
Increased plasma conc w/ erythromycin, INH, verapamil, diltiazem, dextropropoxyphene, viloxazine, possibly cimetidine. Reversible neurotoxic manifestations may occur if combined w/ lithium (rare). Oral anticoagulants, OC, alcohol.
