Cefepime
Indications/Uses
Treatment of infections caused by susceptible bacteria in lower resp tract including pneumonia & bronchopneumonia, lower UTI including pyelonephritis, skin & skin structure infections, severe or complicated gynecology infection, septicaemia, intra-abdominal infections including peritonitis & biliary tract infection. Empiric treatment of febrile neutropenia. Severe pneumonia in childn 2 mth-16 yr. Monotherapy prior to identification of causative organism in patients who are at risk of mixed aerobic-anaerobic infection caused by Bacteroides fragilis.
Dosage/Direction for Use
Adult Mild to moderate UTI 500 mg-1 g IV/IM every 12 hr. Mild to moderate infections other than UTI 1 g IV/IM every 12 hr. Severe infections 2 g IV every 12 hr. Very severe or life-threatening infections 2 g IV every 8 hr. Childn 2 mth-16 yr Do not exceed adult dose. Childn up to 40 kg Pneumonia 50 mg/kg/dose every 12 hr. Empiric therapy for febrile neutropenic patient 50 mg/kg/dose every 8 hr for 7-10 days. All IV administration is given approx 30 min.
Contraindications
Hypersensitivity to cefepime, cephalosporins, penicillins or other β-lactams.
Special Precautions
Discontinue use when allergic reactions occur. Renal impairment, pseudomembranous colitis. Pregnancy & lactation.
Adverse Reactions
Hypersensitivity reactions. GI disturbances. Chest pain, tachycardia. Headache, dizziness, insomnia, paresthesia, anxiety, confusion. Genital pruritus, taste perversion, chills, unspecified moniliasis, oral moniliasis, colitis, anaphylaxis transient leucopenia, neutropenia, agranulocytosis, thrombocytopenia, SJS, erythema multiforme, TEN, toxic nephropathy, aplastic anemia, hemolytic anemia, hemorrhage. Vaginitis, peripheral edema, pain & back pain. Local inj site reactions.
Drug Interactions
Metronidazole, vancomycin, gentamicin, tobramycin sulfate or netilmicin sulfate soln. Concomitant use w/ aminoglycosides, diuretics & furosemides may enhance nephrotoxicity & ototoxicity.
