Indications and Dosage
Topical/Cutaneous
Menopausal atrophic vaginitis
Adult: 0.01% cream. A progestogen is required if used on a long-term basis in women with uterus.
Contraindications
Pregnancy, porphyria, undiagnosed abnormal genital bleeding, known or suspected oestrogen-dependent neoplasia, deep-vein thrombosis or pulmonary embolism, arterial thromboembolic disease, liver disease or impairment, hypersensitivity.
Special Precautions
Conditions that might be aggravated by fluid retention, cerebrovascular or coronary artery disease. Women with strong family history of breast cancer or who have breast nodules, fibrocystic disease, abnormal mammographic findings. Women undergoing surgery, those with fracture or who are immobilized. Renal insufficiency, impaired hepatic function, metabolic bone diseases. May exacerbate asthma, diabetes mellitus, epilepsy, migraine, porphyria, systemic lupus erythematosus, hepatic hemangiomas. Child (premature closure of the epiphyses). Lactation.
Adverse Reactions
GI disturbances, dermatologic reactions, elevated BP (high doses), thromboembolic disorders, fluid retention/oedema, decreased glucose tolerance, hypercalcaemia, folate deficiency, cholestatic jaundice, liver tumors, genitourinary effects, mental depression, dizziness, altered libido, chorea, headache, keratoconus, contact lens intolerance, haematologic effects, breast changes. Discontinue in case of partial or complete loss of vision, proptosis or diplopia, papilloedema, retinal vascular lesions.
Potentially Fatal: Increase risk of gallbladder disease, deep-vein thrombosis, stroke, MI, pulmonary embolism, malignancy.
Lab Interference
Alteration in endocrine and LFT results. Increased serum TG concentrations.

Mechanism of Action: Dienestrol is a synthetic nonsteroidal oestrogen that is structurally similar to diethylstilbestrol.
