Thiamphenicol
Indications/Uses
Infections due to Salmonella sp, H influenzae (especially meningeal infection), rickettsia, lymphogranuloma-psittacosis, gm -ve bacteria.
Dosage/Direction for Use
Adult, childn, babies >2 wk 50 mg/kg in divided doses, to be taken 3-4 times daily. Premature babies, babies <2 wk 25 mg/kg in divided doses, to be taken 4 times daily.
Administration
Should be taken on an empty stomach: Take on an empty stomach 1 hr before or 2 hr after meals.
Contraindications
Hypersensitivity. Not for prophylaxis of bacterial infections or treatment of trivial infections, throat infections & flu. Not for hepatobiliary infections & gonorrhea. Patients w/ impaired liver & kidney function.
Special Precautions
Regular hematological exam should be performed in prolonged use. Treatment should be discontinued if reticulocytopenia, leucopenia, thrombocytopenia, or anemia occurs. Prolonged use. Patients w/ impaired kidney function. Pregnancy, lactation. May cause Grey’s syndrome in newborn babies (0-2 wk) & premature babies.
Adverse Reactions
Blood dyscrasia ie, aplastic anemia, hypoplastic anemia, thrombocytopenia, granulocytopenia; GI disturbances eg, nausea, vomiting, glossitis, stomatitis, diarrhea; hypersensitivity reactions eg, fever, skin rash, angioedema, urticaria; headache, mental depression, optic neuritis & Grey’s syndrome.
Drug Interactions
May cause cross sensitivity w/ chloramphenicol. Dicoumarol, phenytoin, tolbutamide, phenobarb.
