Indications and Dosage

Oral
Chronic haemorrhoids

Adult: Each 500 mg tab contains MPFF equivalent to diosmin 450 mg and hesperidin 50 mg
Each 1,000 mg tab contains MPFF equivalent to diosmin 900 mg and hesperidin 100 mg
Recommended dose: 1,000 mg daily.

Oral
Acute haemorrhoidal attack

Adult: Each 500 mg tab contains micronised purified flavonoid fraction (MPFF) equivalent to diosmin 450 mg and hesperidin 50 mg
Each 1,000 mg tab contains MPFF equivalent to diosmin 900 mg and hesperidin 100 mg
Recommended dose: 1,000 mg tid for the 1st 4 days, followed by 1,000 mg bid for the next 3 days. Seek medical advice if symptoms do not resolve within 15 days.

Oral
Chronic venous insufficiency

Adult: Each 500 mg tab contains MPFF equivalent to diosmin 450 mg and hesperidin 50 mg
Each 1,000 mg tab contains MPFF equivalent to diosmin 900 mg and hesperidin 100 mg
For cases involving the lower limbs (e.g. swollen or heavy legs, early morning restless legs, pain, nocturnal cramps): Recommended dose: 500 mg bid (midday and evening) or 1,000 mg once daily in the morning. Doses must be taken at mealtimes.

Administration

Should be taken with food. Tab: Swallow whole & do not break/crush. Some formulations may be dissolved in water. Consult product literature for specific instructions.

Special Precautions

When used for acute haemorrhoidal attack: Therapy must be short-term; if symptoms do not improve promptly, perform a proctological exam and further review the treatment. Pregnancy and lactation.

Adverse Reactions

Gastrointestinal disorders: Diarrhoea, dyspepsia, nausea, vomiting, abdominal pain, colitis.
General disorders and administration site conditions: Malaise.
Immune system disorders: Angioedema.
Nervous system disorders: Rarely, dizziness, headache.
Skin and subcutaneous tissue disorders: Rarely, rash, pruritus, urticaria.

Overdosage

Symptoms: Diarrhoea, nausea, abdominal pain, pruritus, and rash. Management: Symptomatic treatment.

Action

Description:
Mechanism of Action: Diosmin and hesperidin are venotonic and vascular protective flavonoid compounds which act on the vascular return system. The combination increases venous tone by inhibiting the breakdown and prolonging the activity of norepinephrine, thus reducing venous distensibility and stasis. At the microcirculatory level, it normalises capillary permeability and reinforces capillary resistance. Additionally, it improves lymphatic drainage by decreasing the intralymphatic pressure and enhancing the removal of interstitial fluid.
Diosmin, a naturally occurring flavonoid, may be synthesised from hesperidin or commonly isolated from citrus fruits.

Onset: 2 hours.
Pharmacokinetics:
Absorption: Rapidly absorbed.
Metabolism: Diosmin: Extensively metabolised into phenolic acids.
Hesperidin: Converted by the microflora of the colon to hesperetin.
Excretion: Diosmin: Mainly via faeces (approx 80%); urine (approx 14%). Elimination half-life: 11 hours.
