Levofloxacin (tab:base; infusion:hemihydrate)
Indications/Uses
Acute maxillary sinusitis, acute exacerbation of chronic bronchitis, community-acquired pneumonia, UTI including mild to moderate pyelonephritis, & uncomplicated, mild to moderate skin & skin structure infections.
Dosage/Direction for Use
FC tab Usually 250-500 mg 24 hrly. Acute exacerbations of chronic bronchitis 500 mg daily for 7 days. Community-acquired pneumonia 500 mg daily for 7-14 days. Acute maxillary sinusitis 500 mg daily for 10-14 days. Uncomplicated skin & skin structure infections 500 mg daily for 7-10 days. Complicated UTI 250 mg daily for 10 days. Acute pyelonephritis 250 mg daily for 10 days. Renal dysfunction CrCl 20-49 mL/min Initially 500 mg followed by 250 mg 24 hrly, CrCl 10-19 mL/min, hemodialysis & chronic ambulatory peritoneal dialysis Initially 500 mg followed by 250 mg 48 hrly. Complicated UTI & acute pyelonephritis CrCl 10-19 mL/min Initially 250 mg followed by 250 mg 48 hrly. Infusion Usually 250-500 mg slow IV infusion once daily for not less than 60 min for 7-14 days. CrCl 20-49 mL/min Initially 500 mg followed by 250 mg 24 hrly, 10-19 mL/min & <10 mL/min, hemodialysis & chronic ambulatory peritoneal dialysis Initially 500 mg followed by 125 mg 24 hrly.
Administration
May be taken with or without food: Ensure adequate fluid intake.
Contraindications
Hypersensitivity. Pregnancy & lactation. Childn <18 yr.
Special Precautions
Discontinue in cases of hypersensitivity reaction, occurence of pain, inflammations, rupture of tendon; diabetic patients taking insulin or oral antidiabetic; increase of ICP & CNS stimulation. Renal insufficiency (CrCl <50 mL/min), known or suspected CNS disorders. Diagnose patient w/ diarrhea. Avoid excessive sunlight. Periodic assessment of renal, hepatic, hematopoetic function. May impair ability to drive & operate machinery.
Adverse Reactions
Shock, anaphylactic symptoms, TEN, convulsion, acute kidney failure, jaundice, agranulocytosis, interstital pneumonia, serious form of colitis accompanied w/ bloody stools eg, pseudomembranous colitis & rhabdomyolysis.
Drug Interactions
Antacids, sucralfate, metal cations, multivit. Increased CNS stimulation & convulsive seizure w/ NSAIDs. Antidiabetics may cause blood glucose level disturbances.
